JOB SUMMARY:     Responsible for all required duties in the evaluation and treatment of patients referred to Occupational Therapy to improve function and ADL.  Completes all necessary paperwork required to complete evaluation, treatment, and insurance information on referred patients.

DUTIES & RESPONSIBILITIES: Performs evaluation and develops treatment plan and appropriate goals on all patients referred to Occupational Therapy.  Documents initial evaluation, assessment plan, program, progress notes, and discharge summaries.  Fills out any applicable insurance forms needed.  Educates patients regarding existing problems and treatments being administered when appropriate.  May be asked to meet during lunch or after hours for departmental meetings, in-services, and/or continuing education.  Encouraged to seek and maintain current knowledge and understanding of treatment procedures through continuing education, and participate with sub-committees of SDMG.  Prepares monthly statistics for Occupational Therapy for the director.  Performs other related duties as requested by the Rehabilitation Director.  Must complete OSHA training initially at orientation and annually thereafter.

RELATIONSHIP WITH OTHERS: Direct contact with patients both in person and over the phone.  Must maintain a courteous and effective relationship with patients of the Group.  Patient accommodation and satisfaction is critical.  Must maintain an effective working relationship with co-workers, staff, physicians and administration.

EDUCATION/EXPERIENCE/KNOWLEDGE: B.S. Degree in Occupational Therapy through an accredited program, a New York State license in Occupational Therapy, along with a current registration.

PHYSICAL DEMANDS:

Lifting:            Must be able to lift 20 lbs. infrequently.

Standing:         Must be able to stand as needed to assist patient treatment and perform appropriate evaluation.

Sitting:            Must be able to sit for patient treatment 30 minutes at a time.
