We’re looking forward to your upcoming start date. The Slocum-Dickson marketing team will be marketing your practice. In order to do this, it is very important that you fill out the form below and you schedule your professional headshot with our photographer in advance of your start date.

THE FOLLOWING ITEMS ARE CRITICAL TO MARKETING YOUR PRACTICE AND FILLING YOUR SCHEDULE.

Physician bio info – Please fill this out and return it ASAP. We need this completed months prior to your start date.
Professional photos – IMPORTANT – Please schedule these photos for the first possible date. If you are moving from another state and come to NY to find a home, please have your photos taken while you are here.

Your photo and bio are the keys to successfully launching your marketing campaign. Timing is very important.

Step 2 - Professional Photos

Please schedule an appointment with our Photographer, Kate McCoy, months in advance of your start date (or as soon as possible after accepting your offer). If you visit the area to look for housing, please schedule your photography appointment during that trip. We cannot get these photos soon enough. We do not want to hold up your advertising because we are waiting on photos. This is an important component to your marketing and advertising campaign.

A photography appointment takes approximately 15 minutes in the studio. Once the photos are taken and marketing receives a link to view your online proofs, you’ll be able to select the photo of your choice. Please try to have these taken far in advance of your start date and select your favorites from the proofs in a timely manner.

Tips:

Stay away from busy patterns and logos.
Pretend the camera/photographer is a patient and be yourself.
Wear professional business attire.
If you have your white coat, please bring that as well.

Please note: A traditional headshot and a photo from the knee up will be taken. Please dress accordingly.
