- Purpose of this Policy. This Privacy Policy (“Policy”) of Slocum Dickson Medical Group, PLLC, a professional limited liability company (“SDMG,” “we”, or “us”) applies to all visitors, users, and customers who access or interact with our website and related online services (“you” or “your”). The purpose of this Privacy Policy is to inform you of our Environment (defined below), including the types of personal information we collect, how that information is used, stored, and protected, and the circumstances under which it may be shared. This policy is designed to promote transparency, safeguard user privacy, and ensure compliance with applicable data protection and privacy laws. - About Us. SDMG is a physician-owned and directed multi-specialty group that offers over 25 medical departments and provides quality medical care for patients in Central New York.
- Set of Online Documents. This Policy includes and incorporates our set of online documents, including our Terms of Use and HIPAA Notice of Privacy Practices. Our Terms of Use include and incorporate this Policy. Certain health information that we collect may be considered “protected health information” or “PHI” as defined under the Health Insurance Portability and Accountability Act of 1996, as amended (“HIPAA”). You should review our HIPAA Notice of Privacy Practices, which governs our use and disclosure of PHI.
- Scope
This Privacy Policy covers the collection of personal information obtained through the following list of resources and property, which may vary depending on the nature of your interactions with us and may not include all the examples listed below (collectively, our “Environment”):
- Website https://sdmg.com (the “Site”) as well as any and all additional websites, mini sites, online portals, web portals, mobile applications and electronic user interfaces owned or controlled by us that are incorporated into or connected to the Site. Certain parts of the Site are publicly available; however, in order to use certain other parts of the Site, you may be asked to first complete a registration form and set up a user account with log-in credentials. During registration, you may be asked to give contact details and other demographic information.
- Any of our social media channels and messaging platforms that enable you to communicate with us via emails, texts, or direct messages.
- Any of our phone systems and video conferencing systems that enable you to communicate with us via teleconference, video conference, or video session messaging.

- Minimum Age Requirement
Access to our Environment is not intended for children under 13, and we do not knowingly collect their information. Therefore, all users of the Environment must be at least 13 years old. If you believe a child under 13 has provided personal information, please contact us pursuant to Section 13 of this Policy so we can remove it. Still, parents/guardians or referring providers may share a child’s information to access services—and if you do so, you confirm you are authorized to consent.

When you provide us your email address, phone number, and contact information through registration processes or other mechanisms in our Environment, you consent to SDMG communicating with you through email, text message, phone messages or other forms of communication regarding our available services, the Site, your personal information, and related topics. Certain email messages, text messages, or other communications through social media channels may not be secure and could be intercepted by a third party.
