MyChart Account Update

On October 29th, Slocum-Dickson Medical Group (SDMG) will update MyChart, its patient portal system, through a partnership with Mohawk Valley Health Systems (MVHS). Your records will be transferred to a new version of MyChart and remain accessible to you. Please note the following when logging into your MyChart account beginning October 29, 2025:

  • If you currently have MyChart through SDMG and not through MVHS, you will continue to use the same username and password you currently use to access the new MyChart system.
  • If you have two MyChart accounts, one for SDMG and one for MVHS, you will use your current MVHS username and password to access the new MyChart system.
  • If you have an inactive SDMG MyChart account and no MVHS MyChart account, you will need to create a new account.

Access to SDMG/St. Joseph's MyChart Account information

Access to your older medical information will be available for a limited time. In order to have access to this information you will need to follow the instructions below.

** Do Not Use the SDMG/St. Joseph’s MyChart system to try to reach your SDMG providers as they will not receive the requests. They will only receive requests in the new SDMG/MVHS MyChart account.

Steps:

1.  Click on the three teal, horizontal lines

2. Type “link” in the search bar and click Link My Accounts.

3. Type in “st joes” in the Discover Accounts to Link search bar, then click the Link Accounts button. Follow the directions provided to link your account.
