All patient portal communication will come from Slocum-Dickson, including text messages and emails.
Welcome to Slocum-Dickson Medical Group’s MyChart Patient Portal. This will be your gateway to access your medical information and communicate with your providers in a simple and safe manner.
Slocum-Dickson Medical Group uses Epic Community Connect, an extension of Mohawk Valley Health System’s comprehensive electronic health record (EHR) system.
Signing up is simple
During your office visit you will receive an activation code. Click “sign up for an account” in the web form on this page. After you enter your activation code and some demographic information, you will create your personal user name and password and can begin utilizing the features of MyChart.
