Seeking individuals with certifications and experience in Spine and Manual therapy, vestibular Rehabilitation, Lymphedema, hand therapy and/or Pediatrics.  Work hours would be Monday through Friday, 30 hours per week to begin and potential for 40 hours per week.

JOB SUMMARY: Responsible for all required duties in the evaluation and treatment of patients within the department.  Completes all necessary paperwork required to complete evaluation, treatment, and insurance information on department’s patients.  Must have experience with working with patient’s needing therapy for their hands for strength and conditioning.

DUTIES & RESPONSIBILITIES: Performs evaluation and treatment plan on all patients referred for physical therapy.  Documents initial evaluation, assessment plan, exercise program, progress notes, and discharge summaries.  Fills out any applicable insurance forms needed.  Educates patients with explanation of existing problems, treatment being administered and exercise program when appropriate.  May be periodically asked to meet on evenings or Saturdays for education in-services and/or continuing education courses.  Encouraged to seek and maintain current knowledge and understanding of treatment procedures, continuing education, and participation with sub-committees of SDMG.  May occasionally attend surgery at the discretion of the surgeon and the availability of time as the department allows.  Performs other related duties as requested by the Rehabilitation Director.  Must complete OSHA training initially at orientation and annually thereafter.

RELATIONSHIP WITH OTHERS:  Direct contact with patients both in person and over the phone.  Must maintain a courteous and effective relationship with patients of the Group.  Patient accommodation and satisfaction is critical.  Must maintain an effective working relationship with co-workers, staff, physicians and administration.

EDUCATION/EXPERIENCE/KNOWLEDGE:  B.S. in Physical Therapy and current New York State license in physical therapy, as well as current registration.

PHYSICAL DEMANDS: 
Lifting:  Must be able to lift 20 lbs. infrequently.
Standing: Must be able to stand as needed to assist patient treatment and perform appropriate evaluation.
Sitting: Limited ability to sit required.
