Please note that all patient portal communication will come from “MySt.Joseph’s”, including text messages and emails.
Welcome to Slocum-Dickson Medical Group’s MyHealth Patient Portal. This will be your gateway to access your medical information and communicate with your providers in a simple and safe manner.
Slocum-Dickson Medical Group uses Epic Community Connect, an extension of St.Joseph’s Health comprehensive electronic health record (EHR) system.
Signing up is simple
During your office visit you will receive an activation code. Click “sign up for an account” in the web form on this page. Please note that you will be re-directed to the MySt.Joseph’s website, our Epic Community Connect partner. After you enter your activation code and some demographic information, you will create your personal user name and password and can begin utilizing the features of MyHealth.

Communicate electronically and securely with your care team
*Please note that lab and radiology tests ordered by a provider outside of Slocum-Dickson Medical Group are not able to be viewed through the patient portal.
As a St. Joseph’s Epic Community Connect partner, Slocum-Dickson’s MyHealth patient portal is housed on the MySt.Joseph’s website mystjosephs.sjhsyr.org.
If you see the Slocum-Dickson Medical Group logo in the upper left corner under the MySt.Joseph’s logo, you can be confident you are logging into the correct patient portal. The Slocum-Dickson Medical Group logo will appear on all login pages along with the MySt.Joseph’s logo and the St. Joseph’s Health logo.
